Position Summary :
Indeed Hire and CenterLight are looking for a nurse practitioner who will provide primary medical care as well as care coordination and care management to an assigned panel of Direct CenterLight Healthcare members residing in contracted nursing homes and will work in conjunction with members’ PCP’s, Medical Directors, and IDT. The nurse practitioner will conduct regular, and as needed health risk assessments, and work as part of the Nursing Home Interdisciplinary Team (IDT). The CCS, as the Care Manager, functions as the liaison for the Nursing home Interdisciplinary Care Team and Centerlight Healthcare departments, and NP oversees the departments for addressing and following up the member’s individualized care plan. This position will be located at their facility at 2401 Laconia Ave, Bronx ,NY.
Responsibilities :
  • Provides and coordinates primary medical care to assigned panel of members, including initial Health Risk Assessment upon enrolling in CenterLight Healthcare or on re-admission to the nursing facility after hospitalization. Provides ongoing monthly and annual monitoring with emphasis on early identification of changes in condition, and develop a treatment plan in collaboration with the PCP and IDT
  • Understands and meets the ISNP Model of Care requirements
  • Reviews MDS and Care Area Assessments as part of the comprehensive assessment upon enrollment, when there is a significant change, and/or at least annually and completes HRA.
  • Reviews ICP routinely and participates in the Care Plan meetings
  • NP oversees the appropriate Nursing Home and Centerlight departments for the following up with member’s ICP.
  • Provides medical routine care to ensure management of chronic conditions, using evidence based guidelines. Manages episodic illnesses and changes in condition promptly
  • Stratifies acuity level for all members at least monthly and develops a plan of care based on members’ risk category and communicates same to the IDT at the nursing home
  • Provides interim medical visits in accordance with the policies of Centerlight Healthcare to review and update the medical treatment plan. Formulates treatment plans designed to treat episodic conditions in the facility when possible, in collaboration with the PCP.
  • Orders diagnostic and therapeutic procedures necessary to maintain the members’ health and well-being.
  • Collaborates with Medical Management staff to obtain prior authorizations when necessary. Works with the nursing facility staff as a team member to identify early change in condition and intervene as soon as possible.
  • Is on call for ISNP members in the contracted facilities in rotation with other staff.
  • Care manager role includes but not limited to discussing and explaining to the members or family members the treatment plan of care and provides counseling regarding the members’ medical diagnosis, overall status, and Advanced Care Planning. The NP Reviews Advance Directive options with members or family. Proactively contacts members’ families to keep them aware of members’ condition, at least monthly. The NP provides the support to the members/families during transitions of care
  • As a Care Manager the NP assures that HEDIS and other quality program guidelines are met.
  • Participates in training programs and educational meetings.
  • Supports nursing staff in providing additional care for members who are treated in the facility.
  • Determines, adjusts and consults regarding appropriate evidence based treatment modalities.
  • Provides comprehensive medical history and assessment for high risk members
  • Documents all episodic and chronic diagnoses that are evaluated for all members.
  • As a Care Manager the NP Works closely and communicates with specialists, other clinical departments and ancillary services.
  • All other duties as assigned by ISNP Clinical Manager.
Qualifications:
Education: Graduate of accredited Adult Geriatric Nurse Practitioner program.
Experience:
At least one year experience as a Nurse Practitioner with 3-6 months of experience in geriatric care, preferably in a long term care facility. Ability to work independently to assess members, develop a differential diagnosis and initiate treatment plan in a Long Term Facility.
License/Registration:
New York State RN and NP licenses. Current RN and NP registrations required.
DEA Certificate required.
Current Adult/Geriatric NP Certification from accredited program required.CPR/ BCLS certification.
